Most CCaH programs include a wide spectrum of health and wellbeing services, such as nutritional coaching, functional ability assessments to ensure they are able to remain independent, regular home safety assessments to identify when home modifications might be necessary, a personal emergency response system in case of falls or accidents, medication management services, access to activities and social events , and a smooth transition to skilled care, assisted living or memory care if it becomes necessary.
