Through programs such as Sun Health Care Transitions, specially trained nurses establish contact with families while you’re still in the hospital. Our nurses then work to schedule a series of important follow-up appointments – both in the home and over the phone – during the critical 30-day window following hospital discharge.

By providing a system of regular post hospital discharge follow-up appointments, the Sun Health Care Transitions program is able to provide each and every one of its patients with convenient and effective support and care. In fact, patients engaged in this program have experienced a 3.6 per cent readmission rate, compared to a 20 per cent national average among Medicare beneficiaries. Additionally, the cost savings have proven to be significant, with the average cost of a readmission exceeding $13,000.

For West Valley residents, this program – offered in partnership with Banner Boswell Medical Center and Banner Del E. Webb Medical Center – is free (with a referral from a designated hospital professional), convenient, and offers wonderful peace of mind.
