Life Plan Communities, formerly known as Continuing Care Retirement Communities, offer independent living along with all levels of health services under the umbrella of one community. Sun Health Life Plan Communities includes all three Sun Health locations as well as our Sun Health At Home® program.
In a Life Plan Community, residents have access to high-quality assisted living, memory support, and skilled nursing within the community they already know and love. This allows you to make the most important investment ever – an investment in your future. While there are upfront costs associated with Life Plan Communities and Life Care, the benefits can offer savings for you or your family should your needs change, as well as peace of mind in knowing that you’ll have help from those who know you along the way – your Sun Health Team!
Independent Living
Independent living offers residents the convenience and enjoyment of living in their own private residence at one of our three Sun Health Community locations—all while knowing they have access to a wide range of health services if or when they need it.
Sun Heath At Home assists in keeping members well, active and healthy while living independently in their own residence with coordinated services and the support of a personal wellness coordinator so that they can successfully age in place.
Health Services
Health services provide varying levels of care tailored to the needs of every individual. The health services umbrella encompasses everything from assisted living for semi-independent adults to intensive rehabilitation for those recovering from serious illnesses or hospitalization. Backed by the latest advances in senior health care and a distinct person-first mentality, each health service aims to improve overall health and wellbeing.
