Here are definitions behind the common terms you’ll encounter as you start researching aging in place.
Activities of Daily Living (ADLs)
Activities of daily living, or ADLs as they are commonly known, is a term used to describe fundamental skills performed on a daily basis. This can include everyday activities or basic tasks related to self-care, such as bathing, grooming and eating. ADLs are commonly used as a reference to measure the degree that one could independently care for themselves. When considering aging in place, ADLs will be a common assessment to understand the level of assistance that you may need now and in the future.
Aging in Place
Aging in place is a retirement option in which people choose to remain in their home rather than moving to a retirement community. This is growing in popularity among older adults to maintain a level of independence. If choosing to age in place, you are still able to find support that can assist you through your retirement. Sun Health At Home provides support to make aging in place possible through health coordination with a Wellness Coordinator.
Continuing Care at Home (CCaH)
Continuing Care at Home (CCaH) programs bring the benefits of traditional retirement living to the comfort of your own home. They provide members with the assurance and peace of mind of knowing that they have guaranteed lifetime access to a wide range of health care should their health needs ever change.
Emergency Response System
An emergency response system is a device that can be used to alert medical professionals of an emergency, should there be one, without needing access to a phone. Using GPS technology, you can speak directly to a response agent for assistance with the touch of a button. An emergency response system is provided to every Sun Health At Home member to give members and their families peace of mind.
Health Services
Health services are designed to provide care for individuals who need additional assistance with activities of daily living. This can vary in level of care based on personal circumstances. Sun Health Communities offers members several types of health services including:
- Assisted living
- Memory support
- Skilled nursing
- Rehabilitation
The goal of our health services is to provide personalized assistance for everyday living and to promote a healthier life for all. Sun Health At Home members can seamlessly move to one of our Life Plan Community locations and receive the health services they need in the event that they can no longer remain at home.
Independent Living
Independent living is an option for individuals who can live on their own without assistance. This can occur while aging in place or within a Life Plan Community. The benefit of Sun Health Communities and Sun Health At Home is that they both offer flexibility of independent living so members can have vary levels of support based on their specific needs.
In-Home Assistance
In-Home Assistance is support for medically necessary services provided for CCaH members. This can include services such as medication management, meal preparation and transportation. This can ensure that those choosing to age in place still feel supported through their retirement years.
Life Care
A Life Care agreement provides members peace of mind by knowing they have access to all levels of care, wherever they are, should they ever need it. Life Care defines your financial arrangement so that once you are either a resident or Sun Health At Home member, all levels of care will be provided. This differs from other plans as it promotes care even if your financial needs change by no fault of your own or if you outlive your assets.
Life Plan Community
Life Plan Communities are a retirement living option that encourages an active independent living lifestyle. Residents of life plans communities enjoy the benefits of independent living and a variety of health services all within a socially active community. Sun Health Communities has exceptional amenities and a unique and vibrant lifestyle that encourages residents to live life to the fullest while having access to different levels of care if their needs ever change.
Long Term Care Insurance
Long term care insurance is an insurance policy that helps cover the costs related to long term care. This is commonly used to help fill coverage gaps left by health insurance or Medicare. While many believe Medicare will help pay all medical costs, Medicare only pays for long term care under specific circumstances, such as after a hospitalization, and only for a limited time. In response to this, supplemental long term care insurance plans have grown in popularity, even though they still include limitations and waiting periods.
Sun Health At Home provides an innovative retirement option for members and can also help bridge the gap in an existing long term care insurance policy, as a more comprehensive plan for the future.
Sun Health At Home
Sun Health At Home is Arizona’s first Continuing Care at Home program where you can remain in your own home with the benefits of a community. With a thriving independent living lifestyle and the support of a personal Wellness Coordinator, Sun Health At Home helps members make aging in place a reality. Sun Health At Home focuses on providing and coordinating short or long term care services for members for the rest of their lives. The program also assists in preserving assets when care needs arise and as costs increase in the future.
Wellness Coordinator
A Wellness Coordinator is assigned to each Sun Health At Home member. Their role is to help support members through their retirement years and offer coordination as needed. Wellness Coordinators are able to recommend companies and services as well as coordinate medical appointments and procedures, private duty caregivers, companion services and transportation when necessary. Our team of Wellness Coordinators are available 24/7 and work with members to maintain vibrant health and independence.
