Costs associated with becoming a resident at one of our Sun Health Communities depend on the specific community, residence style and size of the residence. There is a one-time entrance fee and convenient monthly service fee to relieve the stress and burden around paying multiple different bills. Your convenient monthly service fee covers all monthly expenses including dining, housekeeping, groundskeeping, utilities, use of community amenities and more. Life Care defines your financial arrangement so that once you are either a resident or a Sun Health At Home member, levels of care including assisted living, memory support, skilled nursing and rehabilitation are available—even if your financial needs change.
A Life Plan Community is a retirement living option that encourages an active independent living lifestyle, but also offers a range of health services. The community provides exceptional amenities and a unique vibrant lifestyle, allowing residents to live life to the fullest.

Yes, you can bring both. Your independent living residence comes unfurnished, so you can choose to customize your new home by bringing your own furniture and décor. Additional personalization can be done by choosing finishes custom to your taste, such as paint, flooring, cabinets and countertops. However, please note that there may be costs associated with customizations.

Each resident has a designated parking spot to store and park their vehicle(s). This varies across communities but can include garages, underground parking or covered parking.

Yes, of course! Friends and family are always welcome to join residents and enjoy a meal together. In addition, a private dining room may be reserved for special occasions and events.

Yes, Sun Health At Home® is licensed and regulated by the Arizona Department of Insurance. In addition, all staff and service providers are vetted through background checks prior to coming to your home.
Sun Health At Home serves the East and West Valley, which includes the following cities: Anthem, Avondale, Buckeye, Carefree, Cave Creek, El Mirage, Fountain Hills, Goodyear, Litchfield Park, Paradise Valley, Peoria, Phoenix, Scottsdale, Sun City, Sun City West, Surprise, Tolleson, Wickenburg and Youngtown.
The costs associated with becoming a Sun Health At Home member depends on which plan you select and your age at the time you join. There are five different plans to choose from, all varying in coverage, copay and access to community-based health services.
Sun Health At Home members receive services based on their chosen membership plan. However, all plans include a wellness coordinator, health and wellbeing services, access to Sun Health Communities, in-home safety assessments, a personal emergency response system, referral services for vetted vendors and in-home assistance services. Most plans also include facility-based services such as assisted living and memory care.

Once you become a member, you’ll feel the security of knowing that you can continue to live independently, in the home you love, with guaranteed lifetime access to health care should your needs ever change.

A wellness coordinator acts as your partner and personal advocate. They are made available to you 24/7 and ensure that you are prepared, aware of your options and have a plan in place to access the care and services you may need. In addition to coordinating services, your wellness coordinator can become a trusted advocate and communicator to see that all your needs are met.
Sun Health At Home members receive an annual functionality and safety evaluation of their home. This may result in recommended modifications to help you stay safely and independently in your home, for longer.
Sun Health At Home’s overall goal is to keep members living in their own home for as long as possible. However, should your needs change, your wellness coordinator will assist you in finding a retirement community offering assisted living, memory support, skilled nursing or rehabilitation to relocate to. This could include one of our Sun Health Community locations, Grandview Terrace, La Loma Village and The Colonnade, or another community of your choosing.
Members are more than welcome, and encouraged, to join events planned by Sun Health At Home, or to take advantage of the many classes, amenities and dining opportunities at any of the Sun Health Community locations, which are always open to Sun Health At Home members.
